PARENTAL CONSENT

Q: My son is traveling abroad with his friend's family.  I need a letter of consent. Where can I get it? I am divorced and have full custody. Should my husband sign the letter anyway? 

A: Please bring a custody form to the Notary Public along with the letter (We have it in our library). The Notary will attach a copy of the custody form to the letter and  notarize both documents.

If a natural parent is deceased, a certified copy of the death certificate is required.
Step-parents can NOT sign, unless the child has been legally adopted, in which case, legal
documentation supporting this must be attached and notarized.
Please make extra copies of the consent letter to keep with you when you travel!
